Our secretaries are available to set up appointments from 9:00 A.M. until 5:00 P.M. every weekday at

                                       419 885-1910.

In addiition to our regular hours, we do have late afternoon and evening appointments available.

Rates

$135 per 50-minute session

Insurance

Services may be covered in full or in part by your health insurance or employee benefit plan. Please check your coverage carefully by asking the following questions:

·        Do I have mental health insurance benefits?

·        What is my deductible and has it been met?

·        How many sessions per year does my health insurance cover?

·        What is the coverage amount per therapy session?

·        Is approval required from my primary care physician?

Please bring your insurance cards with you to your first appointment.

Payment

Cash, credit cards and checks are accepted and co-pays expected at the time of treatment .

Cancellation Policy

If you do not show up for your scheduled therapy appointment, and you have not notified us at least 24 hours in advance, you will be required to pay one half of the cost of the session unless we can fill the appointment
